Abstract

Kesadaran kesehatan personal merupakan fondasi penting dalam pembentukan perilaku hidup sehat sejak usia dini. Penelitian ini bertujuan untuk menganalisis efektivitas dari implementasi Buku Rapor Kesehatanku terhadap peningkatan kesadaran kesehatan personal peserta didik di SD Negeri Condong IV. Metode penelitian menggunakan pendekatan kuantitatif dengan desain quasi-experimental pre-test dan post-test. Sampel penelitian melibatkan 40 peserta didik kelas V-VI yang dipilih secara purposive sampling. Instrumen penelitian berupa kuesioner kesadaran kesehatan personal dan lembar observasi perilaku kesehatan. Data dianalisis menggunakan uji paired t-test dengan bantuan SPSS 25. Hasil penelitian menunjukkan peningkatan signifikan terhadap kesadaran kesehatan personal peserta didik setelah implementasi Buku Rapor Kesehatanku (p =0,001). Skor rata-rata kesadaran kesehatan meningkat dari 62,48 menjadi 77,27. Aspek yang mengalami peningkatan tertinggi adalah pengetahuan tentang cara menghindari kekerasan seksual (50,19%) serta cara menyikat gigi yang baik dan benar (46,38%). Implementasi buku rapor kesehatan terbukti efektif meningkatkan kesadaran kesehatan personal peserta didik melalui pemantauan berkelanjutan dan evaluasi diri. Program ini dapat dijadikan model pengembangan kesehatan sekolah di tingkat pendidikan dasar.
